What To Bring To Your Appointment

Insurance Card & ID
Please bring these to every visit to ensure proper check-in and insurance processing.

Imaging on a Disk
If you have prior X-rays, MRIs, or CT scans, bring them on a disk for our providers to review.
(We must see the actual images, not just the report, as they provide critical details for diagnosis and treatment.)

New Patient Forms
First-time patients should arrive 15-20 minutes early to complete registration paperwork.

Epidural Injection Guidelines
Preparing for Your Epidural
To ensure a safe and effective epidural injection, it is crucial to follow all pre-procedure guidelines, including stopping certain blood thinners that may increase the risk of bleeding.
Blood Thinner Guidelines For Epidural:
Aspirin & NSAIDs
Stop 5 days before
Plavix (Clopidogrel)
Stop 7 days before
Eliquis, Xarelto, Pradaxa
Stop 3 days before
Coumadin (Warfarin)
Stop 5 days before (physician clearance required.)

In-House vs. Outsourced
We offer custom bracing solutions to support recovery, prevent injuries, and improve mobility. Some braces are available in-house for immediate fitting, while others may be outsourced through trusted medical suppliers, depending on your insurance coverage and medical needs. Your provider will recommend the best option for your condition.
Urine Drug Screens
Guidelines & What To Expect
As part of pain management and compliance protocols, urine drug screenings (UDS) may be required for certain treatments. If you have any concerns about this requirement, please discuss them with your provider before your appointment.

Co-pays are due at the time of service, and all patients must provide their insurance card at registration. We accept most major insurance plans and payment methods for your convenience.
